Payment and Cancellation Policies:

Payment for services rendered are expected at the time of your appointment.  Medical insurance may or may not cover the cost of your appointment.  You are responsible for obtaining reimbursement from your insurance provider.  A detailed receipt of services will be provided at the completion of your visit.

Cash and checks are accepted. 

Cancellation Policy:  Your appointment time is scheduled for you.  Please arrive on time so that you may take full advantage of your scheduled time.  New clients should arrive early so that required forms may be completed.  If you must cancel your appointment, please allow 24 hours notice.  Payment for your appointment is expected for late cancellations and "no shows".

What To Expect At Your Appointment:

Prior to your appointment, you may download a Food Journal and a Registration Form to complete.  

The initial appointment will be spent collecting information for a complete nutritional assessment.  Together, we will investigate your child's growth history, usual food intake, past medical history, personal history and family dynamic with emphasis on behaviors and interactions with food.

Your subsequent appointments will be focused on an individualized plan for your child, nutrition counseling and education.  We will work together to improve the quality of your child's nutrition and health.

Confidentiality:  All information shared during an appointment is kept in strict confidence, compliant with the HIPAA Notice of Privacy Practices.
